Documentation required for enrollment:

Enrollment card, occupational medicine doctor's certificate (for both programs) and other documents with which additional rights for enrollment have been realized.

Note: Exceptionally, a candidate who is unable to submit an occupational medicine medical certificate at the time of enrollment must submit a family doctor's certificate upon enrollment, and submit the occupational medicine medical certificate to the school no later than September 30 of the current school year
